We are seeking a highly motivated and pro-active individual to fulfil the role of Patient Pathway Co-Ordinator within our busy Respiratory team. Your job role will mean that you will build a good working relationship with Consultants, Admin Team, Nursing Staff and external providers. You will need good communication skills, written and verbal, as you are dealing with patients face to face and over the phone.
The post holder will be joining the Respiratory Team based at Southampton General Hospital, Minerva House. The team consists of 12 team members. The hours are Monday to Sunday, 8am-8pm but currently the post holders working hours will be 8am-4pm Monday to Friday.Main duties of the job

What you'll do
Implement all administrative processes along the patient pathway to ensure the patient's journey is managed efficiently, smoothly and in accordance with targets and agreed timescales.
Ensure referral grading outcomes are logged on the appropriate patient information system.
Ensure patient information is maintained and updated on the appropriate patient information system.
Utilise computerised tracking systems to locate investigation results.
What we're looking for
RSA 2 word processing/typing or equivalent experience.
Good standard of secondary education with literacy and numeracy skills demonstrable to GSCE pass level or equivalent standard.
Proficient in all aspects of Microsoft Office/Excel
Diploma level qualification or equivalent experience.
Administrative and organisational experience
