Posting Summary

Rutgers, The State University of New Jersey is seeking a Patient Navigator for the Department of Medicine-Infectious Disease within the New Jersey Medical School. The Patient Navigator provides clients with medical case management services as defined by the Health Resource Services Administration (HRSA).

Among the key duties of this position are the following:

  • Provides client-centered services that link clients with health care, psychosocial, and other services.
  • Coordinates and follows-up to ensure that medical treatments have been provided.
  • Ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client's and other key family members' needs and personal support systems.
  • Ensures that comprehensive medical case management is provided including the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
  • Coordinates/facilitates and follows-up on medical treatments and patient referrals for core medical services and other services as needed.
  • Monitors and tracks client progress and compliance with medical/lab requirements and treatment plan.

    Minimum Education and Experience

  • Bachelor's Degree in a Health, Public Health, Social Work or other relevant field required; plus two (2) years' experience in the development and/or operations of community programs.
  • Equivalent education, experience, and/or training may be substituted for the degree requirements.
  • Certifications/Licenses Required Knowledge, Skills, and Abilities

  • Computer literacy with competence in Microsoft Office components.
  • Must possess excellent written and verbal communication skills; strong organizational skills; detail oriented; able to function independently and work flexible hours; friendly, outgoing, sociable; open-minded & non- judgmental; caring, empathic, compassionate; committed, dedicated, persistent; creative & resourceful.
  • Preferred Qualifications

  • Phlebotomy certification.
  • NJDHSS HIV counselor certification.
  • Knowledge of Newark community.
  • Bilingual skills (English/Spanish).
  • Equipment Utilized Physical Demands and Work Environment

  • Position requires lifting to 10 lbs; on occasion more based upon patient demands. Assist with transportation services in the clinic.

    Immunization Requirements

    Under Policy 60.1.35 Immunization Policy for Rutgers Employees and Prospective Employees, Rutgers University requires all prospective employees to provide proof that they are fully vaccinated and have received a booster (where eligible) against COVID-19 prior to commencement of employment, unless the University has granted the individual a medical or religious exemption.
