Works as a member of team
Assessing community support needs of the consumer by conducting a thorough assessment
Meets with clients on a regular basis determined by clinical necessity
Requests and arranges for services in accordance with treatment and service plans and documents requests for same
Develop client-centered treatment plans (ISP)
Collect history
Coordinate services
Create crisis plans
Create budget and assist client to meet financial needs
Medication management
Work with in-home support, family members, probation/parole, and other members of the treatment team
Follow up to ensure treatment plan is being put into effect
Advocate for clients' unmet needs
Write progress notes for each meeting with client
Responsible for communication with all providers
Enters complete and accurate documentation in clinical records in accordance with mandates
Works with payee or as payee within the scope of practice as defined by SSA
Knowledge of State of Maine's licensing and Maine Care regulations
Must be willing to be flexible with scheduling hours
Requirements:Twenty-one years of age or greater; In good physical and emotional health, as evidenced by satisfactory completion of all background checks and a pre- employment physical.
Qualifications:MHRT-C certification and a minimum of one year experience with persons with mental illness and/or developmental disabilities. Preference to those with current ANSA and LOCUS certificates.
